  4.    The shareware version of this program only allows 20 entries to be made,
  5.    and experimented with. However, all features, including the CHANGE,
  6.    RETRIEVE, ENTER, and PRINT modes, are active and fully functional. Print
  7.    them more than once, to fill a page and use the FULL PAGE auto functions.

  9.    The cost of the CRE-A-TEST program is $15.00 plus $2.00 S/H for each
  10.    disk.  The registered version will allow the entering of 20,000 individual
  11.    questions.  Fill in the following form, cut on the dotted lines, and send 
  12.    it, along with your money order or certified check, (no cash please), to 
  13.    the address that is indicated below.  If you send a personal check, please 
  14.    allow time for your check to clear, before expecting delivery of the 
  15.    program.  See the phone number and credit cards honored below, for credit 
  16.    card purchases.
